1 Purpose
This document defines the assessment records and field requirements for Review Request and Award Mark Use Form Pack within the defined Good Taste Awards standards system.
2 Scope
This document applies to the requirements, decisions, records, controls, and outputs expressly identified under Review Request and Award Mark Use Form Pack. Award scope and exclusions are defined by GTA-STD-100; this document shall not create an additional award dimension. Operational prerequisites identified here affect assessment validity and never constitute product certification.
3 Authority and interpretation
The Data and Publication Custodian owns GTA-FRM-302; the Standards Authority approves it. formal verbs, defined terms, stable requirement identities, policy constants, and generated representations shall be interpreted under GTA-GOV-002, GTA-GOV-005, and the defined registers.

4 Requirements
GTA-FRM-302-R001 — Review request
The Data and Publication Custodian shall capture requester, product, challenged matter, basis, requested remedy, and supporting evidence.
GTA-FRM-302-R002 — Admissibility
The Data and Publication Custodian shall classify procedure, calculation, scope, comment, claim, publication, or unsupported disagreement.
GTA-FRM-302-R003 — Review evidence
The Data and Publication Custodian shall capture records examined, conflicts, reconstruction, findings, and limitations.
GTA-FRM-302-R004 — Review decision
The Data and Publication Custodian shall capture uphold, correct, recalculate, replace, reassess, lower, withdraw, restore, or close outcome.
GTA-FRM-302-R005 — Mark-use request
The Data and Publication Custodian shall capture party, product, star, channel, artwork, adjacent claims, and intended scope.
GTA-FRM-302-R006 — Use review
The Data and Publication Custodian shall capture identity, wording, visual, digital, accessibility, and comparative-claim checks.
GTA-FRM-302-R007 — Permission
The Data and Publication Custodian shall capture asset, permitted scope, restrictions, responsible party, and state.
GTA-FRM-302-R008 — Misuse
The Data and Publication Custodian shall capture observed use, evidence, classification, correction, communication, verification, and closure.

6 Completion and interface rules
The Data and Publication Custodian owns completion control. A digital or printable implementation shall preserve field IDs, labels, applicability, allowed values, instructions, and verification states. Conditional fields shall become visible before the user can create a contradictory state. A validation message shall identify the failed rule and the correction required without deleting the original entry.
The final view shall separate supplied facts, calculated values, professional judgments, authorization, and declarations. Restricted identity shall not appear in a blind operational view. Closure shall generate an immutable form-state event and a content integrity value.
7 Conformity and records
Conformity with Review Request and Award Mark Use Form Pack requires evidence comprising the field values, source references, validation results, authorization, and correction lineage. The record for each applicable GTA-FRM-302 requirement shall permit reconstruction without relying on an unrecorded explanation.
Evidence created under GTA-FRM-302 shall use stable identifiers, immutable event sequence numbers, and machine event timestamps. A correction shall be append-only. A nonconformity shall place the affected object in a non-releasable state until its consequence and confirmed disposition are recorded. Role combination remains subject to the mandatory separation rules.
